Premature ejaculation: Pharmacotherapy vs group psychotherapy alone or in combination

Objectives: Premature Ejaculation (PE), the commonest sexual dysfunction in males, is generally treated with local anesthetic and SSRI (Dapoxetine). The aim of our study was investigate Group Psychotherapy as an alternative treatment for PE and compare the efficacy of pharmacological treatment and psychotherapy, either alone or in combination, in terms of response and improved Quality of Life (QoL). From a male outpatient population screened for PE, those who received a diagnosis of PE were proposed for the study, enrolled and divided into 3 groups (A, B and C). Alpha-adrenergic drugs in retrograde ejaculation.

6 men with complete or partial loss of ejaculation as a result of retroperitoneal lymphadenectomy (performed because of testicular tumor) were treated with an intravenous injection of 60 mg synephrine. Synephrine stimulates adrenergic alpha-receptors. Only 1 man with sustained retrograde ejaculation improved following treatment showing antegrade ejaculation. This temporary restoration of fertility was thought to be due to an increase in bladder neck tone and prevention of backflow of semen into the bladder as a result of stimulation of adrenergic alpha-receptors.

Introduction Premature ejaculation (PE) is a sexual dysfunction with high prevalence. According to some reports, it is present in about 20-30% of the male population. Since 2009 PE has been treated with a novel inhibitor of serotonin re-uptake, Dapoxetine, which has been reported to be specifically active for PE. This study aimed to investigate how the morphology of cortical and subcortical brain structures differed in PE, how these morphologic differences were associated with severity measures of PE, such as intravaginal ejaculatory latency time (IELT), and how these cortical and subcortical structures were causally connected through mediation analysis.
